Episcopal Community Services, ECS offers home health care as well as pastoral care training and support. ECS Home Care provides certified home health aides for low-income homebound elderly. ECS home health aides provide personal care, and caregiver respite support. ECS also offers pastoral training and support in hospitals, nursing homes and churches in Bucks, Chester, Delaware, Montgomery, and Philadelphia Counties. Life Assess Program
Holy Redeemer LifeAssess is the first program of its kind in the nation to be certified by the Joint Commission on the Accreditation of Healthcare Organizations (JCAHO). The LifeAssess program -- a collaborative effort of Holy Redeemer's two home health agencies is a comprehensive assessment and rehabilitation program for people age 85 that seeks to identify and offer intervention for the four most common geriatric syndromes (falls risk, incontinence, depression and dementia). Out Respite Program: Temple University Center for Intergenerational Learning
College students are trained to assist caregivers with elderly relatives living in Philadelphia. Bilingual students are available for Latino families. Services include: caring companionship, meal preparation, providing a safe environment, assistance into the bathroom, lending support to caregivers, recreational and social activities, light grocery shopping and escort to medical services. The cost is $7.00 per hour and there is a $25.00 annual registration fee. Services
are scheduled at least 8 hours per month and the same student is assigned for 14 weeks.

County programs are designed to support the caregiver so they can
continue to provide care to an older loved one in the care recipient's
home.
National PACE (Programs for All-inclusive Care for the Elderly) Association
Programs for All-inclusive Care for the Elderly provide and coordinate
all needed preventive, primary, acute and long term care services so that an older individuals can continue living in the community. To be eligible for PACE a person must be 55 or older, be certified by the state to need nursing home care and live in an area served by a PACE program. In Pennsylvania, this program is called LIFE (Living Independently for Elders), due to the prexisitng pharmacy
benefit program administered by the state also called PACE. Further information on PACE and LIFE programs can be found at the national web site.

The Telesupport Project
Telephone-based support for women aged 55 years of age and older caring for a relative with Alzheimer's Disease or other dementia. Research project conducted by em>Thomas Jefferson University and the Supportive Older Women's Network (SOWN), funded by the Alzheimer's Association. Purpose is to find the best ways to provide support to women caregiving for a relative for Alzheimer's Disease or other dementia. Participants are paid $30.00.

The Visiting Nurse Association of Greater Philadelphia is a not-for-profit provider of home health care throughout southeastern Pennsylvania. VNA services include skilled nursing, therapies, social work and home health aide care as well as home hospice care and specialty care services. VNA HouseCall, a team of visiting doctors and nurse practitioners, provides diagnostic, prescriptive and primary care medical services in patients' homes.

Community Choice is a statewide program of the New Jersey Department of Health and Senior Services created to assure that senior citizens, persons with disabilities and their families are aware of choices they have when it comes to long term care.
